The below Liability Waiver, Media Release and Privacy Policy are integral policies of Inspire Dance Company LLC (“Inspire Dance Company”), which pertain to (“Student”) and Student’s participation in the exercise and/or dance programs of Inspire Dance Company. By signing below, I agree to adhere to and be bound by each of the policies outlined below.
LIABILITY WAIVER
I understand that risk of injury is inherent in any physical activity and I, on behalf of myself and Student, knowingly and voluntarily accept any such risks involved in participating in the exercise and/or dance program of Inspire Dance Company
LLC.
I, the undersigned, for myself, my heirs, Student, administrators, and executors, hereby waive and release Lauren Lombardi individually and Inspire Dance Company LLC- including but not limited to its owners, agents, facilities, volunteers, assistants, employees, guest artists, faculty members, and/or students- from any and all claims or damages of any kind arising out of Student’s participation in the exercise and/or dance program of Inspire Dance Company.
Furthermore, I agree to obey the class and facility rules and take full responsibility for mine and Student’s behavior in addition to any damage that I or Student may cause to the facilities and business of Inspire Dance Company.
I further certify that Student is in proper physical condition to participate in the exercise/dance program. I, the undersigned, do hereby authorize Inspire Dance Company and its designated agents to obtain medical treatment for Student in emergency situations where I cannot be reached in time to authorize the treating physician to provide such emergency medical services.
I understand the importance of updating the medical information and details for Student via this portal, or in writing and commit to ensuring Inspire Dance Company and Student's teachers will be aware of any or all medical issues concerning Student.
I understand that I am responsible for any medical expenses that may come about in connection with Student’s participation in Inspire Dance Company’s programs. This authority includes the power to authorize any and all treatment deemed necessary under the circumstances by a licensed physician. This power shall remain in effect for the entirety of Student's involvement/enrollment in classes, workshops, or any events held at / in conjunction with Inspire Dance
Company.

MEDIA RELEASE FOR INSPIRE DANCE COMPANY
Students of Inspire Dance Company may have the opportunity to be photographed or videotaped at various dance events.
Pursuant to this release, any photos, videos or other media featuring Student may be included for Inspire Dance Company publications or advertisements, including on Inspire Dance Company’s website and social media profiles.
To ensure privacy, specific names of children will not be included.
No financial compensation shall be given for use of any photographs or video featuring Student.
I acknowledge that Inspire Dance Company may choose to use photos, videos or other media featuring Student at its own
discretion. Inspire Dance Company reserves the right to discontinue use of photos, videos or other media without notice.
I have read this release and I understand its contents. I consent to and authorize the use by Inspire Dance Company, or anyone authorized by Inspire Dance Company, of any and all photographs, video or other media which have been taken
of Student.

When you register for classes at Inspire Dance Company, we collect contact information such as guardian names, addresses, phone numbers, emergency contacts, e-mail addresses and student information such as student names, birthdays, medical conditions, and your comments. Some of the information is required. This information is needed to manage studio business and will not be used or sold for any other purpose, except as described below. WHO HAS ACCESS TO MY INFORMATION?
When you register, you are required to enter an email address and a password. The combination of that email account and password is used to ensure that only you have access to your/your child/children’s personal information through Inspire Dance Company Parent Portals in Jackrabbit. Our office personnel have access to your personal information only to manage the studio business.
Inspire Dance Company personnel will access your personal information and/or share it with third parties only for the following purposes: (i) as needed for studio business; (ii) if required to do so by law or to comply with legal process; or (iii)
to protect the rights, property, or personal safety of the public.
