I, the parent or legal guardian of the above-named child, understand that participation in Brazilian Jiu-Jitsu, wrestling, and martial arts training involves physical activity and inherent risks, including but not limited to falls, contact with other participants, strains, sprains, and other injuries.
I knowingly and voluntarily allow my child to participate in the BJJ Takedown & Wrestling Camp and assume full responsibility for any risk of injury, illness, or loss that may occur as a result of participation.
I hereby release, waive, and discharge the camp organizers, instructors, coaches, staff, affiliates, and facility from any and all liability, claims, demands, or causes of action arising out of or related to my child's participation, including those resulting from negligence, to the fullest extent permitted by law.
I certify that my child is physically able to participate and that I have disclosed any relevant medical conditions or injuries. I understand it is my responsibility to notify instructors of any changes to my child's health.
Photo & Video Release:
I grant permission for my child to be photographed or recorded during camp activities. I authorize the use of photos and videos for promotional, educational, marketing, and social media purposes without compensation or prior approval.
By signing below, I confirm that I have read, understand, and agree to this waiver in full.
